Member Reference Desk contains downloadable forms and documents for your health plan. Once you enter your group number and subscriber number, you will have access to documents that may include your benefit summary, member handbook, certificates of coverage, advance directives, privacy statements, pharmacy mail order forms, and prescription drug lists.
