University of Michigan Health Plan (UM Health Plan) has built a network of providers who are committed to providing quality care to our members, and we encourage like-minded healthcare professionals in UM Health Plan’s service area to consider partnering with us.

If you are a provider located in one of the counties UM Health Plan serves and would like to be considered for participation status, please complete the New Provider Request Form and submit it as directed.

Currently, UM Health Plan has a sufficient number of providers in some specialties, including but not limited to: laboratory, chiropractic, durable medical equipment, outpatient surgical centers, optometrists, physical therapy and behavioral health management services.
