At University of Michigan Health Plan (UM Health Plan), we work hard to help you get the coverage, care, and personal support you deserve, without all the hassle.
That means ensuring you have access to the care prescribed by your health care providers to achieve your health care goals.
Our case management program steps in to help following an inpatient hospital stay, at the request of your provider, or for those members with complicated health situations.
What can you expect?
A case manager will call you to discuss your health care needs. Together with you, your family, and your doctors, we will formulate a plan to meet your health care goals and we will follow up to ensure they are met. We advocate on your behalf to find resources and connect you with the services you need, including:
- Medications
- Home Health Care
- Rehabilitation Services
- Finding a Primary Care Provider (PCP)
- Referrals when seeking care from an Out-of-Network Specialist
- Education on your condition
- Community resources
- Transplant support
- Behavioral Health
- Substance Use Disorder (SUD) Support
Who can benefit?
You may benefit from our case management program if you:
- See several health care providers for your condition
- Take multiple medications
- Have questions about your condition or treatment plan
- Have questions about how to access providers or resources to help meet your health care goals
