How to Access Medicare Documents & Forms

Medicare documents and forms must be accessed through the Medicare Advantage Portal.

1. Login to the Provider Portal.
2. Find the PHP Medicare Portal link located under the Office Management drop-down.

3. 

   Find the document or form you're looking for in the Forms & Resources link on the far right.
4. Contact UM Health Plan at 844-529-3757 if you require assistance.
